If you have Insurance, please bring your membership card with you. If you are unsure of your coverage, we suggest that you contact your carrier prior to your appointment or visit. If we are not providers of your insurance we would be pleased to give you a copy of your receipt so you can self submit to your insurance or flex spending benefits program.
